THE GAP

The medication half of care management

Care management programs need someone to do the medication work: full-regimen review, interaction and duplication checks across prescribers, prescribing cascades, deprescribing candidates, and follow-through with the patient and family after a change is ordered. Hiring a pharmacist means salary, benefits, and a hiring cycle. Contracting one starts in weeks and scales with your panel.

How the Arrangement Works

Contracted clinical staff

I furnish clinical staff time within my scope of practice, under your general supervision.

Your practitioners decide

I recommend in writing. Your practitioner decides. I carry approved changes to the patient and document what happens.

Your practice bills

Your billing team selects the codes. I document time and work to the standard your program requires.

Built for compliance review

Business Associate Agreement before any record is shared, a secure HIPAA-compliant portal, and a care plan built collaboratively and signed by your billing practitioner.

Services for practices

Patient Review

$250 per review. Your first review is at no cost.

A full medication and supplement review for one patient, with written recommendations built to be acted on inside a short visit. The lowest-commitment way to see the work on your own charts.

Program Build

$2,500, one time

The foundation for a pharmacist-supported care management program: a written protocol drafted to include consent script, care-plan template, enrollment workflow, time-documentation standard, and an annual protocol review calendar.

THE CORE ARRANGEMENT

Monthly Medication Management

Flat monthly fee by enrolled panel size

Enrolled patients Monthly fee

Up to 75. $2,400

76 to 150. $3,900

151 to 300. $6,500

Every month I review each enrolled patient's full regimen, send your prescribers written recommendations, and carry approved changes to patients and families.

Hourly consulting

$199 per hour

Provider education, protocol work outside a retainer, and expert medication consults.

Pharmacogenomic testing with interpretation is available through an accredited laboratory partner where a result would change a decision.

How we start

1. An introductory call about your panel, your care management programs, and where medication work is falling through.

2. Your first patient review, at no cost, so your team can see the work on a real chart.

3. A written proposal sent after the call, so your team can review it on its own schedule.
