Twelve prescriptions.
Four prescribers.
One list
nobody has read start to finish.

When several specialists each manage one condition, nobody looks at the whole list. Whether the medications are yours or a parent's, I review every prescription, supplement and over-the-counter product with you, and give you a written plan you can actually understand.

CARE CONSULTATION

Two Paths

Whether you're sorting out your own medications, helping a parent with theirs, or looking for pharmacist support for your practice, I shape the work around your situation.

For individuals and families

For you, or for a parent you help care for. I review every prescription, supplement and over-the-counter product, check for interactions and duplicates, and work alongside your doctors to simplify daily routines and bring clarity to the care plan.

For clinics and care teams

Pharmacist-delivered medication management for the patients in your practice who need it most, furnished as contracted clinical staff. First patient review at no cost.

The Systemic Gap

Why nobody has done this already

Patient POV: Your cardiologist manages your heart rhythm. Your rheumatologist adjusts your joint therapy. Your primary physician balances your metabolic labs. Each clinician makes sound, responsible decisions within their specialty, yet none are scheduled or compensated to step back and evaluate the cumulative burden of 5 (or more) overlapping prescriptions. Modern outpatient medicine is organized around episodic diagnosis rather than continuous regimen stewardship, which means medication lists tend to expand over time without anyone having the dedicated hours to simplify them.

Provider POV: Care management programs need someone to do the medication work: full-regimen review, interaction and duplication checks across prescribers, prescribing cascades, Deprescribing candidates, and follow-through with the patient and family after a change is ordered. Hiring a pharmacist means salary, benefits, and a hiring cycle. Contracting one starts in weeks and scales with your panel.

SERVICES

Start with the whole list

The Medication Clarity Workup is where individual work begins. I read your full medication list before we meet, walk through it with you live, and hand you a written plan to bring to your prescriber.

Coaching for what comes next. After your review, I can coach you through the next steps, whether that's a taper, a conversation with a prescriber, or learning to read your own list with confidence.
