When several prescribers each manage one piece of care, nobody looks at the whole list. I review every prescription, supplement and over-the-counter product, and give you a written plan you can actually understand. Free 15-minute call by video. No commitment.
Your mom's cardiologist manages her heart. Her rheumatologist adjusts her joint therapy. Her primary doctor watches her labs. Each one makes sound decisions in their own area, but no one is asked to step back and look at everything together. That gap is where side effects get treated as new conditions, and where medications outlive the reason they were started.
A short video call to talk about what's going on and whether a full review is the right fit. No cost and no commitment.
After a secure intake form, I read your full list before we meet. Then we go through every medication, supplement and over-the-counter product together by video, each sorted against a current reason for taking it.
You receive a plan you can read and understand for yourself. With your permission, I share the findings with your family and prescribers, and can coach you through what comes next: a taper, a talk with your prescriber, or reading your own list with confidence.
I work alongside your doctors, not around them. Medication changes stay with your prescriber. Some reviews end with keeping everything exactly as it is: the goal is right, not fewer. This isn't emergency care; in an emergency, call 911.
$447 · 75-minute video consultation
A review of every prescription, supplement and over-the-counter product. A check for interactions, duplicates and medications without a current reason. A written plan you can read and understand, and, with your permission, findings shared with your prescribers.

I'm a licensed pharmacist focused on medication review, deprescribing and coaching for adults taking several medications from several prescribers. I work with patients, their families and their prescribers, sharing findings and the plan so everyone works toward the same goal.
It is a careful, independent review of the whole list, explained in plain language.
It is collaborative. I work alongside your doctors, not around them.
